Consistently, magnesium intake was significantly inversely associated with hs-CRP, IL-6, fibrinogen, and HOMA-IR, and serum magnesium levels were inversely correlated with hs-CRP and HOMA-IR. Conclusions Magnesium intake was inversely longitudinally associated with incidence of diabetes in young American adults. This inverse association may be explained, at least in part, by the inverse correlations of magnesium intake with systemic inflammation and insulin resistance. I think, as you hypothesize, that many insulin-resistant "lean" people would actually turn out to be "skinny-fat" if you put them in a DXA or MRI machine. If you look at insulin sensitivity vs. BMI and consider only younger people who don't have illness-induced weight loss, people in the very lean BMI category (20 and below) almost all have high insulin sensitivity. I think that's because when BMI goes that low, there is not much room for adipose tissue. Those people are truly lean. However, they did show that insulin resistance was an independent predictor of events even after controlling for BMI. So presumably, a person's risk of events would be higher if they are insulin resistant, regardless of whether that person is lean or overweight. Of course, an overweight person is more likely to be insulin resistant.<br /><br />Regarding your question of whether any insulin-resistant lean people were identified, I don't know for sure, but the answer is almost certainly yes.
